Compass Disability Network:
 

We need to collect certain personal data to enable Compass Disability Services/CDST to manage your membership of Compass Disability Network, including attending events, forums and associated meetings.

In order for us to take bookings, payments and register attendance we will ask you to give us certain personal data including children’s data (if children are attending events or sessions) either directly or via our website and booking platform.

Compass Disability Services/CDST is the Data Controller for data that we receive from you. Some of the data we hold is considered special category data (details about a disability, age etc) processed under General Data Protection Regulations Article 9 condition D.

We need to collect certain personal data to enable Compass Disability Services/CDST and our delivery partners (Mind Taunton & West Somerset, South Somerset Mind and Chard Watch CIC) in order that we can support our customers.

Compass Disability Services/CDST is the Data Controller. This notice applies to customers, supporters and professionals associating with the service. We also collect some special category data which is processed under Article 9, condition H of the General Data Protection Regulations (GDPR).

Personal data that is processed by SMWS includes the following:

• Name, residential address and contact details including telephone numbers and email address

• Date of birth

• Details about your mental and/or physical health

• Details of our interactions with you

Compass Disability Services/CDST has a legitimate interest and will process and control such data principally for the purposes of delivering the Somerset Mental Wellbeing Services including support groups, workshops and general administration.

In order for us to support you with the management of your direct payment (health or social care funded) we will ask you to give us certain personal data.

Compass Disability Services/CDST is the Data Processor for data that we receive from your funder (local authority and/or CCG) about your care. We are the Data Controller for data that we collect from you in order to support you with ongoing management of your care package, payroll and/or managed account services. Some of the data we hold is considered special category data processed under General Data Protection Regulations Article 9 condition H.

Personal data that is processed in order to provide you with payroll services includes the following:

Your name, residential address and contact details including telephone numbers and email address

• Your date of birth

• Some demographic information such as your age, ethnicity or first language

• Some financial details, such as your National Insurance Number, bank account,

• Your employees name, residential address and contact details including telephone numbers and email address

• Your employees date of birth

• If you receive funding on behalf of someone else because you are their appointed person or because they are a child/young person then we may also hold information about them.
