Compass Disability Network:
 

We need to collect certain personal data to enable Compass Disability Services/CDST to manage your membership of Compass Disability Network, including attending events, forums and associated meetings.

Compass Disability Services/CDST is the Data Controller. This notice applies to members and professionals associating with the service. We also collect some special category data which is processed under Article 9, condition D of the General Data Protection Regulations (GDPR).

Personal data that is processed by Compass Disability Network includes the following:

  • Name, residential address and contact details including telephone numbers and email address.

  • Date of birth.

  • Details about your mental and/or physical health to enable us to meet your access needs.

  • History of our interactions e.g. attendance registers, emails etc.

Compass Disability Services/CDST has a legitimate interest and will process and control such data principally for the purposes of delivering the Compass Disability Network including forums, groups, workshops and general administration.

We will seek your permission to send you general information about our charity, events and information. You can update your permission at any time by contacting us.

Your details will not be shared beyond the scope of this notice. Anonymised data will be used for statistical analysis and reported to commissioners. We may ask you if we can use some non-specific comments or questions you have raised, or anonymised details about your situation as case studies in our reports.

Compass Disability Services/CDST does not accept responsibility or control of data willingly shared by you with any other third party independent of and outside of Compass Disability Services/CDST control.

Compass Wellbeing:

In order for us to take bookings, payments and register attendance we will ask you to give us certain personal data including children’s data (if children are attending events or sessions) either directly or via our website and booking platform.

Compass Disability Services/CDST is the Data Controller for data that we receive from you. Some of the data we hold is considered special category data (details about a disability, age etc) processed under General Data Protection Regulations Article 9 condition D.

Personal data that is processed in order for you to access our services, sessions and events includes the following:

Name, residential address and contact details including telephone numbers and email address

  • Date of birth  (for children)

  • Disability/health issues that we might accommodate

  • We do not directly store you payment details as this is done through our booking platform Booking Bug. Booking Bug is a non-UK based company and as such your data will be held securely outside of the E.U.

We may periodically send you updates about the service we offer, events that we are hosting etc that may be of interest to you.

Somerset Mental Wellbeing Service (SMWS):

We need to collect certain personal data to enable Compass Disability Services/CDST and our delivery partners (Mind Taunton & West Somerset, South Somerset Mind and Chard Watch CIC) in order that we can support our customers.

Compass Disability Services/CDST is the Data Controller. This notice applies to customers, supporters and professionals associating with the service. We also collect some special category data which is processed under Article 9, condition H of the General Data Protection Regulations (GDPR).

Personal data that is processed by SMWS includes the following:

  • Name, residential address and contact details including telephone numbers and email address

  • Date of birth

  • Details about your mental and/or physical health

  • Details of our interactions with you

Compass Disability Services/CDST has a legitimate interest and will process and control such data principally for the purposes of delivering the Somerset Mental Wellbeing Services including support groups, workshops and general administration.

If we have concerns about your safety or the safety of others, we may share the details with others.  Please see our safeguarding policy for full details, which is available at www.compassdisability.org.uk

Your details will only be shared with the organisations delivering the SMWS. Anonymised data will be used for statistical analysis and reported to commissioners. We may ask you if we can use some non-specific details about your situation as case studies, but we will ask for your consent separately and explain who will see the case studies.

They are not allowed to use your personal data for their own purposes. Compass Independent Living Services:

In order for us to support you with the management of your direct payment (health or social care funded) we will ask you to give us certain personal data.

Compass Disability Services/CDST is the Data Processor for data that we receive from your funder (local authority and/or CCG) about your care. We are the Data Controller for data that we collect from you in order to support you with ongoing management of your care package, payroll and/or managed account services. Some of the data we hold is considered special category data processed under General Data Protection Regulations Article 9 condition H.

Personal data that is processed in order to provide you with payroll services includes the following:

Your name, residential address and contact details including telephone numbers and email address

  • Your date of birth

  • Some demographic information such as your age, ethnicity or first language

  • Some financial details, such as your National Insurance Number, bank account,

  • Details of your care package funding and purpose

  • Your employees name, residential address and contact details including telephone numbers and email address

  • Your employees date of birth

  • Some financial details of your employees, such as their National Insurance Number, bank account, tax code, deduction from earnings, student loan repayment details and statutory payment history

  • If you receive funding on behalf of someone else because you are their appointed person or because they are a child/young person then we may also hold information about them.

Personal data that is processed for managed account services includes the following:

  • Your name, residential address and contact details including telephone numbers and email address

  • Your date of birth

  • Some financial details including bank account details, personal contribution amounts

  • Case notes

  • Some demographic information such as your age, ethnicity or first language

  • Details of your care package funding and purpose

  • Your employees name, residential address and contact details including telephone numbers and email address

  • Some financial details of your employees, such as bank account details, tax code, deduction from earnings and student loan repayment details

  • Financial details regarding suppliers you use including names, addresses and bank details of self-employed people.

  • If you receive funding on behalf of someone else because you are their appointed person or because they are a child/young person then we may also hold information about them.

Personal data that is processed for ongoing support services includes the following:

  • Your name, residential address and contact details including telephone numbers and email address

  • Your date of birth

  • Some demographic information such as your age, ethnicity or first language

  • Details about your health and/or disability

  • Case notes

  • Any known risks with working with you

  • Some financial details including bank account details, personal contribution amounts

  • Details of your care package funding and purpose

  • Your employees name, residential address and contact details including telephone numbers and email address

  • Some financial details of your employees, such as bank account details, tax code, deduction from earnings and student loan repayment details

  • Financial details regarding suppliers that you use including names, addresses and bank details of self-employed people.

  • If you receive funding on behalf of someone else because you are their appointed person or because they are a child/young person then we may also hold information about them.

We will periodically send you legal/service updates about the service we provide, your obligations and messages from your local authority/CCG.

Compass Disability Services/CDST has a legitimate interest and will process and control such data principally for the purposes of administration of your support for your health or care funding. Data may also be used where Compass Disability Services/CDST is under a legal obligation to do so or where it is necessary and proportionate to safeguard the interests of staff, customers and/or the public interest.

Compass Disability Services/CDST may receive and share data with the following parties in order to assist them in the management of your support and fulfil the purpose set out above:

  • HMRC

  • Pension providers

  • Insurance Company

  • Safeguarding Teams

  • Local authority

  • CCG

  • Managed Account Providers

They are only allowed to use your personal data for the purpose for which it was collected.
